The Florida Department of Health has published a standardized form for physicians to certify that they have determined an individual to be extremely vulnerable to COVID-19 and eligible to receive the vaccine.

The form was published Tuesday evening. It will be used across Florida beginning Wednesday.
Late Friday, the governor signed an executive order allowing extremely vulnerable people to get the vaccine.
Doctors will fill out the form and patients will take it with them to get a vaccine.
